Does resveratrol really activate sirtuins?
Here's where the story gets honest. Resveratrol does activate SIRT1 in the lab — but the mechanism turned out to be more complicated than the early headlines. Detailed studies showed that resveratrol's activation is substrate-specific: it depends partly on the chemical "tag" attached to the test molecule, which sparked years of debate about how much of the original lab signal reflects real biology versus an artifact of the assay (Hou et al., 2016). The current understanding is that resveratrol can stabilize SIRT1's interaction with certain substrates — a real effect, but a subtler and more conditional one than "resveratrol switches on your longevity genes."
What's the catch with resveratrol itself?
Bioavailability. Resveratrol is absorbed and then cleared from the body rapidly, so very little circulating, active resveratrol reaches your tissues from an oral dose. The combination raised whole-blood NAD+ by roughly 40% at the recommended dose and about 90% at double dose, sustained over 8 weeks, with no serious adverse events (Dellinger et al., 2017). That establishes the combination is safe and that the NAD+-raising effect (driven by the precursor) holds up alongside a polyphenol. What it does not establish is that adding the polyphenol produces benefits beyond what the precursor alone would.

And the counterweight that the marketing skips. Resveratrol doesn't always help — sometimes it interferes. In a controlled trial, 27 inactive older men did 8 weeks of exercise training while taking either 250 mg/day of trans-resveratrol or placebo. Exercise improved cardiovascular markers in the placebo group, but resveratrol blunted those gains, including the improvement in aerobic capacity (Gliemann et al., 2013). The likely explanation is that exercise benefits depend partly on transient oxidative stress, and resveratrol's antioxidant action dampened that signal. The honest summary: the biochemistry is reasonable, the safety of precursor-plus-polyphenol combinations looks acceptable, and the proof of added benefit is missing. If you're drawn to the combination for a specific health reason, that reason deserves a medical conversation rather than a supplement bet — particularly because resveratrol can interact with medications (it can affect how some drugs are metabolized and may influence bleeding risk).
