Short answer: in most people over 50 it is scaffolding failure rather than blood failure — but the exceptions are the reason you should read the pattern carefully. The small vessels running through your dermis are supported by surrounding connective tissue. When cumulative ultraviolet exposure degrades that tissue, the vessels lose their mechanical backing and rupture from contact you would not previously have noticed. Your platelets and clotting factors may be entirely normal. What changed is the packaging.
What actinic purpura actually is
It goes by several names — senile purpura, solar purpura, Bateman's purpura — and was first described in 1818, when Thomas Bateman worked out that the dark purple blotches on older patients' forearms were blood that had leaked into the dermis.
The lesions are sharply demarcated purple macules or patches, typically on the dorsal hands and extensor forearms. They are painless and non-inflammatory. They resolve over roughly one to three weeks, often leaving a brownish-yellow stain from haemosiderin, and they do not scar. A distinctive feature is that patients frequently cannot recall any injury at all — the triggering trauma was too minor to register.

How common is it, really?
Common enough that most people over 60 will encounter it. Dermatology reviews put the prevalence at approximately 11.9% of adults older than 50, with prevalence rising markedly with years of sun exposure. It occurs with roughly equal frequency in men and women, which is itself informative — it points to exposure rather than hormones as the driver.

Dermatologists increasingly place it within a broader syndrome called dermatoporosis — chronic skin fragility of ageing, with senile purpura, stellate pseudoscars and skin atrophy as its recognised markers. The framing is deliberate: it draws an analogy with osteoporosis, arguing that skin fragility is a definable condition worth preventing rather than an inevitability worth ignoring. The same reviews note that easy bruising and actual skin tears travel together in older adults, which is why this is a falls-and-injury issue as much as a cosmetic one.
One immunohistochemical study of the microvasculature in senile purpura found increased basement membrane thickness around dermal vessels compared with young controls — the opposite of the intuitive prediction. The authors suggested this might represent a compensatory response. It is a useful reminder that the mechanism is not fully settled.
Patterns that need a clinician rather than reassurance
What actually helps
Photoprotection. This is the intervention aimed squarely at the cause. It will not undo decades of accumulated damage, but it addresses the process that is still running. Long sleeves on the forearms do more than most creams.
Topical retinoids. These are the pharmacological agent most consistently cited in dermatology reviews for improving dermal resilience in this specific condition, on the reasoning that thickening the dermis restores some vascular support. It is worth discussing with a clinician rather than self-prescribing, since retinoids irritate fragile skin.
Mechanical protection. Unfashionable and effective. Long sleeves, protective sleeves for gardening or pet handling, padding on furniture edges, and moving hazards out of habitual walking routes reduce the trauma that produces the lesions in the first place. Given the documented link between senile purpura and skin tears, this is a genuine injury-prevention measure.
What the evidence does not support. Oral supplements marketed for bruising — including citrus bioflavonoids, which appear in consumer guidance — have a thin evidence base for this indication. Vitamin K creams are widely sold with limited controlled data. We would not spend money there before addressing UV exposure and mechanical protection.
What this won't do
Recognising actinic purpura does not rule out anything else. The two can coexist: an older adult with classic sun-related forearm purpura can simultaneously develop a low platelet count or start an anticoagulant. Pattern recognition is a starting point for a conversation, not a substitute for one. If your bruising has changed in character, frequency or distribution, that change is the signal — not the individual bruise.
Frequently asked questions
Why do my bruises last so much longer than they used to?
Two reasons. Clearing extravasated blood depends on local circulation and immune cell activity, both of which are slower with age — the same slowdown covered in our piece on why wounds heal more slowly. And haemosiderin staining can persist after the bruise itself has resolved.
I take a daily aspirin. Is that causing this?
It can contribute, by making any vessel rupture bleed more before it seals. But aspirin does not cause the underlying dermal atrophy. Discuss it with your prescriber — do not stop antiplatelet or anticoagulant medication on your own, since the reason you were prescribed it has not gone away.
Does easy bruising mean I have a vitamin deficiency?
Usually not. Severe vitamin C or K deficiency does cause bleeding, but both are rare in people eating an ordinary diet, and the forearm pattern of actinic purpura does not suggest them. If there is genuine concern, a blood test settles it far more cheaply than a supplement trial.
Will it get worse?
Prevalence rises with age and cumulative exposure, so on average yes. Whether it worsens for you depends heavily on ongoing sun exposure and how much minor trauma your hands and forearms take — both of which you have some control over.
Is there any treatment that actually thickens the skin back up?
Topical retinoids are the option with the most support in dermatology reviews, and the goal is improving resilience rather than full restoration. Expect a modest effect, and get advice before applying them to already-fragile skin.
