Is dry eye actually the main cause?

Researchers surveyed 425 community-dwelling adults with an average age of 69 using a validated light-sensitivity questionnaire, after excluding anyone with diagnosed photophobia or eye disease. Most respondents landed toward the lower end of the sensitivity scale, but higher scores were associated with poorer sleep health and lower physical and mental health ratings, which the authors suggested makes a case for screening for light sensitivity as part of routine geriatric assessment rather than treating it as an isolated nuisance complaint.1
A separate line of research on non-visual light responses adds an important honest caveat: the changes are not uniform, and some older adults show almost no measurable difference from younger adults. A study comparing melatonin suppression, alertness, and brain wave responses to evening light exposure in young versus older adults found that while young participants showed clear, consistent light responses, older participants as a group showed no significant differential response — but the effect size varied considerably between individuals, and a meaningful subset of older adults responded much like younger participants.2 In other words, age is a strong statistical predictor of reduced light responsiveness, but it is not a guarantee, and individual variation is large enough that "getting older" alone does not fully explain any one person’s experience.
What increased light sensitivity does not explain
Age-related light sensitivity is typically mild, gradual, and affects both eyes evenly. It is not the same thing as sudden or severe photophobia, which has a different and sometimes urgent differential.
