What does the poison-control data actually say?
A CDC analysis of the National Poison Data System counted more than 260,000 pediatric melatonin ingestions over the decade from 2012 to 2021, with the annual number rising about 530%. By 2021, melatonin accounted for nearly 5% of all pediatric ingestions reported to poison centers, up from 0.6% in 2012. Most ingestions — around 94% — were unintentional, and roughly 83% of children remained asymptomatic.
A separate CDC report estimated about 11,000 emergency-department visits for unsupervised pediatric melatonin ingestion between 2019 and 2022, a period when sales were climbing fast. When symptoms did occur, they were most often central-nervous-system effects (drowsiness), with gastrointestinal and cardiovascular effects less common. Hospitalizations were uncommon, but the report flagged that serious outcomes had increased, and the earlier analysis noted five children required mechanical ventilation and two died.
Why are gummies such a big part of this?
Because they look and taste like candy. CDC investigators found that flavored gummy formulations featured heavily in unsupervised-ingestion cases, and that in most emergency visits a solid dosage form was accessed by a child five or younger. Melatonin products are not currently required to use child-resistant packaging, and when container type was recorded, the bottle had usually been opened or left improperly closed. A child who finds a bottle of fruit-flavored gummies has little reason to stop at one.
There’s a second wrinkle: what’s actually in the gummy. A JAMA analysis of 25 melatonin gummy products found 22 were inaccurately labeled, with actual melatonin ranging from 74% to 347% of the declared amount — and one product contained no melatonin at all, only CBD. How toxic is melatonin, really?
In context: most pediatric ingestions caused minimal or no effects, and a poison-center study found that 98% of pediatric melatonin exposures resulted in minimal or no effects. Melatonin is not a corrosive or a classic high-lethality poison. But “usually mild” is not “harmless,” especially for the youngest children and at the unpredictable doses that mislabeled gummies can deliver. The honest framing is that the risk per ingestion is low, but the sheer volume of ingestions means real harm shows up at the population level.
