Short answer: NEAT is the energy you burn doing everything that is not sleeping, eating, or deliberate exercise — and it varies between people far more than either metabolism or workouts do. It is the most underrated term in the energy balance equation and the one people have the least direct access to. Understanding it explains a lot about why two people with similar diets and similar gym habits end up in very different places.
What exactly counts as NEAT?
Non-exercise activity thermogenesis covers the energy expended for everything we do that is not sleeping, eating, or sports-like exercise. Walking to work, typing, doing yard work, standing at a counter, carrying groceries, shifting in your chair, tapping a foot. Even trivial physical activities raise metabolic rate measurably, and it is the cumulative effect of thousands of small movements across a day that produces the total.
The scale is the surprising part. NEAT can vary between two people of similar size by as much as 2,000 calories per day, driven by differences in occupation and leisure-time activity. That is a larger spread than most people's entire exercise budget.
What did the landmark study actually find?
The overfeeding experiment is the foundation of the field. Sixteen healthy volunteers were fed 1,000 calories per day in excess of their weight-maintenance requirements for eight weeks. Fat gain varied roughly ten-fold across the group. Two-thirds of the increase in total daily energy expenditure was attributable to non-exercise activity thermogenesis rather than basal metabolic rate or postprandial thermogenesis.

The individual numbers tell the story better than the average. Across the sixteen participants, the change in NEAT ranged from about −98 calories per day — one person actually moved less when overfed — to about +692 calories per day. The correlation between the change in NEAT and resistance to fat gain was 0.77, which is a strong relationship by the standards of human metabolic research.
Fidgeting alone is thermogenically meaningful. A separate study used high-precision indirect calorimetry in 24 subjects to measure the energy cost of low-grade movement. Compared with resting supine, energy expenditure rose about 4% sitting motionless, 13% standing motionless, 54% fidgeting while seated, and 94% fidgeting while standing. Walking at a slow 1.6 km/h raised it 154%. The authors concluded that the thermogenic potential of fidgeting-like and low-grade activities is great enough to substantively contribute to energy balance.
The variance between people is enormous. That same study noted marked variation between subjects in the energy expended on self-selected fidgeting. Some people simply move a great deal without registering it as activity; others sit remarkably still. This is not a moral trait, and there is no evidence it responds to being told about it.
Why NEAT matters more than it seems
Most people think about energy balance in two boxes: what I eat, and what I do at the gym. NEAT is a third box, larger than the gym box for nearly everyone, and mostly invisible.
Consider the arithmetic. A demanding hour-long workout might cost 400 to 600 calories. A desk job versus a job on your feet can differ by considerably more than that across a day, every day, without anyone calling it exercise. This is part of why daily step count tracks with health outcomes so consistently even when formal exercise is accounted for — steps are largely a proxy for NEAT.
It also reframes a common frustration. When people say their metabolism slowed down, the measured resting metabolic rate change over midlife adulthood is smaller than most assume. The more likely culprit is a quiet contraction of incidental movement — a shorter commute, a desk role, less manual work at home, fewer stairs — accumulating over years without ever registering as a decision.
The honest counterweight: NEAT is not a lever you simply pull
The mechanism that regulates NEAT is not well understood. Physiological studies show NEAT is modulated with changes in energy balance — it rises with overfeeding and falls with underfeeding — which strongly suggests it is under substantial biological rather than volitional control. Hypothalamic factors that specifically increase NEAT have been identified in animals. As one researcher put it, spontaneous physical activity may not be spontaneous at all.
That has an uncomfortable implication. If NEAT falls automatically when you eat less, then part of the expected deficit from dieting is silently offset by moving less — more slumping, fewer unnecessary trips upstairs, less fidgeting. People generally do not notice this happening.
It also means the honest version of "just move more incidentally" is limited. You can deliberately restructure your environment — a standing desk, walking meetings, parking further away — and those choices are real. But the automatic component that separated the fat-resistant from the fat-gaining in the overfeeding study was not something participants chose. The study was also small, with sixteen people, and has not been replicated at scale.
What you can actually do about it
The realistic strategy is structural rather than motivational. Changes to your environment and routine produce NEAT reliably; intentions to fidget more do not.
Practical levers with a plausible basis: taking stairs by default, standing or walking during phone calls, breaking up prolonged sitting with brief movement, walking for short errands, and choosing parking or transit stops that build in a walk. None of these feel like exercise, which is exactly the point — they do not draw on the willpower budget that formal training does.

Incidental activity does tend to contract across adulthood, though it is difficult to separate biological aging from life circumstances like desk work and reduced physical labour. This drop is probably a bigger contributor to midlife weight change than the relatively modest measured decline in resting metabolic rate.
Does a standing desk make a real difference?
Standing motionless raises energy expenditure only about 13% over lying down, so standing alone is a modest effect. The larger benefit comes from what standing enables — shifting weight, stepping about, moving more freely — which measured considerably higher in calorimetry.
