Nettle sting for chronic knee pain, randomized pilot study — null result. Researchers ran a randomized, controlled, patient-blind pilot specifically to see if the thumb-joint result would extend to knee osteoarthritis, the joint most commonly affected by arthritis. It did not: nettle sting was not shown to be superior to the control for chronic knee pain in this pilot. This is an important honest counterweight — the positive result is specific to small hand joints close to the skin, not a general "nettle cures arthritis" finding.
Darvishi et al., 2020 (Journal of Ethnopharmacology) — oral extract, knee OA. What nettle won't do
Even the positive topical trial only ran for one week per condition and measured a small, superficial joint. There's no evidence nettle — topical or oral — slows structural joint damage or works reliably on larger, deeper joints like the hip.
