Why is NAD+ even part of the hangover conversation?
When your liver processes alcohol, it runs two main reactions. Alcohol becomes acetaldehyde (the toxic intermediate behind much of how awful you feel), and acetaldehyde becomes acetate. Both steps require NAD+ as the electron acceptor, converting it to NADH in the process.
Drink enough and you tilt the cellular NAD+/NADH ratio heavily toward NADH. That shift ripples outward: it suppresses other metabolic pathways, contributes to the fat accumulation and low blood sugar that can follow heavy drinking, and is one mechanistic thread in why alcohol leaves you depleted [1].
So the reasoning behind NAD+ hangover marketing is: alcohol drains NAD+, therefore replacing NAD+ should help. It's a tidy story. The problem is that tidy stories about NAD+ have a track record of not surviving contact with actual trials.
What human studies actually show
On hangovers specifically: nothing. There is no published randomized controlled trial testing NAD+, NR, NMN, or NAD+ IV therapy against placebo for hangover symptoms. Every confident claim you've seen is extrapolation from biochemistry or from anecdote, not from a controlled comparison.
What the broader NAD+ trials tell us is sobering for the hype. Even where NAD+ precursors reliably do their basic job — raising blood NAD+ levels, which NR clearly does in controlled trials [2] — that biomarker change hasn't consistently produced the felt-better outcomes people hope for. When NR was tested for long-COVID fatigue in a 2025 randomized trial, it didn't significantly beat placebo on fatigue [3]. The pattern across the field is that "raises NAD+" and "makes you feel better" are not the same finding, and you can't assume one from the other.
That's the honest counterweight to the drip-clinic pitch: the one thing NAD+ supplements demonstrably do (move a blood marker) is not the thing you actually care about the morning after (feeling human again).
What actually helps a hangover
The unglamorous answers still win. A hangover is driven by dehydration, disrupted sleep, acetaldehyde exposure, inflammation, and often mild blood-sugar dips — so rehydrating, eating, and sleeping address the real drivers. The only fully reliable prevention is drinking less in the first place. Pacing drinks, alternating with water, and not drinking on an empty stomach all genuinely reduce next-day misery in ways no infusion has been shown to match.

What NAD+ won't do

NAD+ IV therapy gets marketed especially aggressively for hangovers, and it's worth understanding why that should raise your guard rather than lower it. An IV bypasses digestion, which sounds impressive, but it doesn't create evidence where none exists — and infusions carry their own small risks and real costs. The fact that something is delivered by a drip in a clinic setting can lend it an air of medical legitimacy it hasn't earned through trials.
Some of what people feel better from after a "hangover drip" is almost certainly the saline itself — rehydration genuinely helps — plus rest and time.
