Does Fish Oil (Omega-3) Actually Help Joint Pain?

Short answer: the evidence is genuinely mixed, tilting toward modest-or-none for pain specifically. Omega-3 fatty acids have well-established anti-inflammatory signaling in the body, and some trials show real, if small, improvements in osteoarthritis pain. But the largest and most rigorous recent trial found no benefit over placebo, which is an important honest wrinkle in a category that gets marketed with more confidence than the data supports.

Why would fish oil affect joint pain at all?

The long-chain omega-3 fatty acids in fish oil — EPA and DHA — compete with the omega-6 fatty acid arachidonic acid for the same enzymes that produce inflammatory signaling molecules (prostaglandins and leukotrienes). Shifting that balance toward omega-3s is thought to modestly dial down inflammatory tone throughout the body, including in and around joints affected by osteoarthritis, which has an inflammatory component even though it's primarily a mechanical, wear-related disease.

What do the strongest trials show?

The best case for fish oil comes from Stonehouse and colleagues (2022), a 6-month, multicenter, randomized, double-blind, placebo-controlled trial in 235 adults with mild-to-moderate knee osteoarthritis. Four grams per day of krill oil (providing about 0.6 g EPA and 0.28 g DHA) produced significantly greater improvement in knee pain, stiffness, and physical function than placebo at 6 months Stonehouse W, Benassi-Evans B, Bednarz J, et al. The honest counterweight is a larger, more recent trial: Laslett and colleagues (2024), published in JAMA, randomized 262 patients with knee osteoarthritis, significant pain, and MRI-confirmed joint inflammation to 2 g/day of krill oil or placebo for 24 weeks. The result: krill oil did not improve knee pain compared with placebo. The authors concluded these findings do not support krill oil for knee pain in this population Laslett LL, Scheepers LEJM, Antony B, et al. Adding to the complexity, a separate randomized trial comparing high-dose (4.5 g/day) to low-dose (0.45 g/day) fish oil in 202 patients with knee osteoarthritis found the low-dose group had somewhat greater improvement in pain and function at the 2-year mark — the opposite of what a simple "more omega-3 helps more" story would predict Hill CL, March LM, Aitken D, et al. What fish oil won't do

No trial here shows fish oil reversing joint damage or preventing osteoarthritis progression on imaging. And given that the largest, best-designed recent trial found no pain benefit at all, it would be dishonest to promise fish oil will meaningfully change your day-to-day knee pain. If your pain is accompanied by swelling, warmth, or fever, that points toward something an omega-3 supplement isn't designed to address — see a doctor.

Dosing, if you try it

Trial doses ranged from roughly 0.45 g/day up to 4.5 g/day of combined EPA/DHA equivalents, most commonly evaluated over 6 months. Given the mixed results, there's no dose that's been reliably shown to guarantee a joint-pain benefit — if you're taking fish oil, it's reasonable to think of general cardiovascular and metabolic support as the primary rationale, with joint comfort as an unproven bonus rather than the main reason to take it.

How fish oil compares to other inflammation-focused ingredients

And compared with a mechanistically different anti-inflammatory option like turmeric/curcumin, fish oil's evidence base for osteoarthritis pain specifically is arguably weaker and more mixed, even though both ingredients share a general anti-inflammatory rationale. This is a useful reminder that "anti-inflammatory" as a category doesn't guarantee a specific ingredient will show up as effective once it's actually tested against placebo in a rigorous trial.

What about fish oil for other forms of joint pain, not just osteoarthritis?

Most of the strongest trial evidence for fish oil and joint symptoms actually comes from rheumatoid arthritis research rather than osteoarthritis — a systematic review of fish oil in rheumatoid arthritis reported reduced NSAID use among patients, and RA is a more clearly inflammatory condition than osteoarthritis, which is primarily mechanical and degenerative. That distinction may partly explain why fish oil's results in osteoarthritis trials look more mixed: an anti-inflammatory supplement may have more to work with in a disease that's driven more directly by inflammation in the first place.

A note before you consider a supplement: Omega-3 fish oil at higher doses can have a mild blood-thinning effect. If you take a blood thinner (like warfarin) or an antiplatelet medication (like aspirin or clopidogrel), or you're scheduled for surgery, talk to your doctor or pharmacist before starting a fish oil supplement.
