So is a fixed wake time actually more important than a fixed bedtime?

This is where honesty matters more than a satisfying answer: no published large-scale trial has isolated "fixed wake time, variable bedtime" against "fixed bedtime, variable wake time" as two separate experimental arms. The regularity research measures the whole pattern, not one anchor point in isolation.

That said, there's a practical reason sleep clinicians tend to emphasize wake time specifically: you can set an alarm and get up regardless of how tired you feel, but you generally can't force yourself to fall asleep on command. A fixed wake time is the lever you actually control, and it has a knock-on circadian effect — waking at the same time each day helps anchor your body clock's light exposure, which in turn makes it easier for your body to feel sleepy at a consistent bedtime on its own. In that sense, wake time often functions as the anchor that bedtime consistency follows from, even though both matter.

Does this hold up even if you get enough total sleep?

This is the finding worth calling out because it cuts against a lot of casual sleep advice. In the UK Biobank analysis, sleep regularity predicted mortality risk independently of sleep duration — irregular sleepers who were getting a normal 7 to 9 hours a night still had worse outcomes than regular sleepers getting a similar amount, on average (Windred et al., 2024; Full Windred et al. 2024, SLEEP). A separate Northern Finland Birth Cohort study following adults for 10 years found irregular bedtimes were associated with roughly double the risk of major adverse cardiac events compared with regular bedtimes, independent of how long people slept.

The honest counterweight: this is observational, cohort-level evidence, not a randomized trial. Nobody has run a large trial that randomly assigns people to "regular" versus "irregular" sleep schedules for years and watched what happens to their health outcomes — for good reason, since that would be difficult to justify ethically. The authors of the follow-up commentary on this research explicitly note that well-controlled trials are still needed to establish a causal relationship, and that irregular sleepers may differ from regular sleepers in other ways (shift work, stress, social patterns) that also affect health.

What consistency won't fix

A rigid schedule doesn't override sleep debt, undiagnosed sleep apnea, or a genuine circadian rhythm disorder.
