What does “circadian melatonin” actually mean?

The 0.5 mg dose advanced the clock by about 3 hours and the 3.0 mg dose by about 3.9 hours, compared with roughly 1.7 hours on placebo. The notable finding: the small dose did almost as much as the larger one, and with a tighter, more predictable response.

What human studies actually show

Low-dose, well-timed melatonin shifts the clock in delayed sleepers. In a double-blind randomized trial, adults with delayed sleep-wake phase disorder took 0.5 mg melatonin timed to their circadian phase alongside evening dim light and a gradually advancing sleep schedule. The combination advanced circadian and sleep timing — and worked whether the dose was timed to a measured or an estimated DLMO.

Timing relative to bedtime matters in larger trials too. In a separate randomized clinical trial in delayed sleep-wake phase disorder, melatonin taken about an hour before a behaviorally set bedtime, combined with sleep-wake scheduling, improved objective and subjective sleep. Notably, post-treatment DLMO didn’t differ much between groups in the measured subset — a reminder that “it helped me sleep” and “it moved my clock” are not always the same thing.

Melatonin is not a substitute for the most powerful clock-setter available to you — light exposure — and taking it at the wrong time can shift your clock in the wrong direction.
