Why NR label quality is so bad — and what that means for buyers

Nicotinamide riboside is an expensive, chemically finicky ingredient that degrades over time, especially in humid or warm conditions. That degradation problem, combined with limited regulatory enforcement of dietary supplement label accuracy, has created a serious quality gap in the NR category.

In February 2025, Niagen Bioscience (formerly ChromaDex, the company that holds the primary NR patents) published results from a market surveillance analysis of 39 NR-containing consumer products purchased from Amazon and direct-to-consumer websites. Only five products (13%) met or exceeded their label claims. Fourteen products had NR content below the method detection limit — meaning they contained essentially no detectable NR despite label claims. An additional 15 contained some NR, but well below the stated amount. Seven counterfeit products were also identified, five of which contained no NR whatsoever. Separately, an ongoing ConsumerLab review program has flagged wide variability in NR potency across the broader market, noting that only 26 of 70 NR supplements tested were near their label claim.

The practical implication: if you buy an untested NR supplement based on price or packaging alone, there is a meaningful probability you are buying a product with little to no active ingredient. That's not a theoretical concern — it's what independent lab data shows.

What is nicotinamide riboside and why does the dose form matter?

NR is a form of vitamin B3 that the body converts into NAD+, a coenzyme central to cellular energy metabolism and DNA repair. NAD+ levels decline with age — by roughly 50% between the ages of 20 and 50 by some estimates — and NR is one of two NAD+ precursors (alongside NMN) that have been studied in human clinical trials. The most important dose form distinction is between nicotinamide riboside chloride (NR-Cl) and other forms. NR-Cl is the most stable and the form used in virtually all published human trials. Some cheaper products use other salt forms or degraded material — the label may list "nicotinamide riboside" without specifying the chloride form. When reading a supplement facts panel, look for nicotinamide riboside chloride specifically.

What human studies actually show

Trammell et al. (2016, Nature Communications, PMCID PMC5062546) — The landmark pharmacokinetics study. Twelve healthy adults received single oral doses of 100 mg, 300 mg, or 1,000 mg NR in a crossover design. Blood NAD+ rose dose-dependently across all three doses within 8 hours. One individual receiving 1,000 mg saw a 2.7-fold increase in blood NAD+ from a single dose. The authors also identified NAAD (nicotinic acid adenine dinucleotide) as a sensitive biomarker of NR-driven NAD+ repletion.

Dellinger et al. (2017, npj Aging, PMCID PMC5701244) — A randomized, double-blind, placebo-controlled trial in 120 healthy adults aged 60–80 comparing NRPT (NR + pterostilbene) vs. placebo over 8 weeks. The recommended-dose NRPT group (containing 250 mg NR) showed approximately 40% increased whole-blood NAD+; the double-dose group showed roughly 90% increase. No serious adverse events were reported. This was the first RCT demonstrating sustained, dose-dependent NAD+ elevation with oral NR.

Conze et al. (2019, Scientific Reports, PMCID PMC6611812) — An 8-week RCT in overweight but otherwise healthy adults, testing 100, 300, and 1,000 mg NR daily. NAD+ rose dose-dependently by 22%, 51%, and 142% respectively and remained elevated throughout the study. No flushing was reported in any dose group, and adverse event rates were similar between NR and placebo groups — a critical finding distinguishing NR from niacin (which causes flushing via GPR109A activation; NR does not activate this receptor).

The honest counterweight: Raising blood NAD+ does not automatically produce measurable clinical benefits. The 2023 Freeberg et al. review (J Gerontol A, PMID 37068054) synthesized human NR and NMN trials and concluded that while NAD+ elevation is consistent and reliable, "the clinical evidence that raising NAD+ concentrations can improve physiological function is unclear." A 2025 double-blind, placebo-controlled trial in 58 long-COVID patients (Davis et al., eClinicalMedicine, PMID 41357333) found that NR significantly increased NAD+ within 5 weeks but did not significantly improve cognition, fatigue, sleep, or mood versus placebo over the trial period. Exploratory within-group analyses showed some signals at 10 weeks, but the primary outcomes were null. This is what honest coverage looks like: the mechanism works; the downstream benefits need more and larger trials.

What to look for on a label — the five-point checklist

1. Ingredient form: nicotinamide riboside chloride. This is the form used in clinical trials and the most stable for long-term storage. If the label just says "nicotinamide riboside" without specifying the salt, it's a yellow flag.

2. Third-party testing with a certificate of analysis (COA). Look for testing by an ISO 17025-accredited laboratory. Recognized programs include NSF Certified for Sport, Informed Sport, USP, and Alkemist Assured. The COA should confirm potency (mg of actual NR per serving) matches label claims. If a brand doesn't publish its COA or can't produce one on request, move on.

4. GMP-certified manufacturing. Current Good Manufacturing Practice (cGMP) certification, ideally from an FDA-registered facility, provides baseline assurance that what's on the label was actually put in the capsule under controlled conditions.

5. Transparent sourcing of the raw ingredient. A small number of raw material suppliers (ChromaDex/Niagen Bioscience, EffePharm, and a few others) produce validated NR-Cl. Brands that disclose their supplier — or use a patented, clinically studied ingredient — are generally more trustworthy than those that don't.

What NR supplements won't do

No NR supplement has been shown in a well-powered human RCT to definitively reverse aging, restore youthful energy in healthy adults, or cure any disease. The preclinical data in mice is striking; human translation has been slower and more mixed. If you're experiencing significant fatigue, cognitive decline, or other symptoms, the cause may have nothing to do with NAD+ — and supplementing without investigating the underlying issue means delaying useful answers. Dosing and timing: what the trials used

The majority of published human trials tested 500–1,000 mg NR per day, typically split into one or two doses taken with food. A half-life of approximately 2.7 hours for circulating NR (observed in pharmacokinetic studies) suggests splitting doses between morning and evening may produce more stable NAD+ elevation than a single large dose, though head-to-head comparison data on dosing schedules in humans is limited. Most trials ran for 6–12 weeks, and participants who sustained supplementation throughout showed maintained NAD+ elevation for the trial duration.

What about liposomal NR?

Liposomal delivery formats claim to improve NR absorption by encapsulating it in phospholipid spheres that survive gastric digestion. Mouse data suggests this can improve tissue distribution of NR — one 2024 study (ScienceDirect) showed improved brain delivery via IV liposomal NR-Cl in a rodent stroke model. However, there is no published head-to-head human RCT comparing oral liposomal NR to standard NR capsules for blood NAD+ elevation. Standard oral NR-Cl is already well-absorbed in humans across multiple trials — it's worth asking whether the delivery premium is justified by existing evidence. Does NR cause the flushing that niacin causes?
